Managing the health information of an entire family can be overwhelming, especially when appointments, medications, and medical history must be tracked for each member. This Health Tracker provides an easy-to-use solution to keep all the important health-related information in one place.

How to use it:

You have 5 essential databases that are linked so that you only have to fill in the basic information once and then the template will help you complete the medical record:

• Patients: here you have the cards to fill in the basic information for each patient.
• Upcoming Appointments: here you can fill in the date, patient's name, speciality, doctor's name, institution and whether the appointment status is complete, scheduled, rescheduled or cancelled. You will also be able to change the view to see the appointments for each patient.
• Medications: here you can fill in all the details of the medicines you are taking. Also, if you fill in the number of doses and how often you take them, you will be notified of the status of how many doses you have left.
• Doctors and Institutions databases: here you can fill in all the contact details of your doctors and the medical institutions where you receive care.
• Procedures: here you can fill in the details of all the medical procedures that have been performed on each of your patients.
